3 TRAUMATISMOS: EPIDEMIOLOGIA
PRIMERA CAUSA DE MUERTE –en la infancia- Etiología del Politraumatismo Infantil, en España: Accidentes de Tráfico: 1º Como Pasajeros 2º Como Peatones 3º En Bicicletas Accidentes en el Domicilio o entorno doméstico . Caídas Agresiones Quemaduras Causticaciones Accidentes por Juegos y Deportes (Colegios, Pistas, Canchas etc.) Violencia Callejera: *Con arma blanca, *Con arma de fuego, IMPORTANCIA DEL REGISTRO NACIONAL DE TRAUMATISMOS EN LA INFANCIA.

4 ASISTENCIA AL POLITRAUMATIZADO
EN EL LUGAR DEL ACCIDENTE: ABC 1º Selección de los vivos y “Rescate”. 2ºLiberación de Vía Aérea (A) 3ºProtección del Ráquis; 4ºAsistencia Ventilatoria (B) y Cardíaca (C) 5º Control Primario de las Hemorragias. DURANTE EL TRASLADO: Directo al centro de Politrauma/neurocirgíay con mejora del soporte ventil. y vasc. EN URGENCIAS DEL HOSPITAL: repetir ABC 1º Mejorar la Asistencia Ventilatoria y cardíaca 2º Mejorar via Venosa Central 3º Diagnósticos de urgencia (Shock, Coma etc....) 4º Diagnostico de las lesiones y orden de prioridades.

5 ASISTENCIA AL POLITRAUMATIZADO
DIAGNOSTICO Y TRATAMIENTO DEFINITIVOS, TRAS EL INGRESO: 1º, del TCE; -2º, del Trauma Torácico, -3º, del Trauma Abdominal y Pélvico; - 4º, del Trauma Facial; -5º, de las Fracturas y de los Vasos Periféricos. -6ºOtros viscerales o periféricos (Ojos, Heridas, scalps etc.) SIEMPRE MANTENIENDO LAS CONSTANTES VITALES SEGUIMIENTO Y TRATAMIENTO DE LAS SECUELAS

10 PRIORIDADES QUIRÚRGICAS
1.-Compresión encefálica grave: Craneostomía 1 ó 2.-Taponamiento cardíaco o rotura de grandes vasos: evacuación o toracotomía 1 ó 2 ó 3.-Tórax abierto/hemoneumotórax: drenaje-toracotomía 4.-Hemorragia abdominal/pélvica con shock: Laparotomía 5.-Compresión medular: Descompresión raquídea 6.-Hematoma dural: Craneotomía y drenaje 7.-Lesión vascular periférica: Ligadura/Sutura 8.-Fracturas abiertas: Limpieza y fijación 9.-Heridas de cuello con afectación visceral: Exploración/reparación 10.-Fracturas Maxilofaciales y otras cerradas: Fijación/reducción 11.-Otras heridas menores: limpieza y sutura

TRAUMATISMOS EN PACIENTES PEDIÁTRICOS -BASES DEL TRATAMIENTO “CONSERVADOR EN ALGUNOS TRAUMATISMOS VISCERALES ABDOMINALES: -Edad: biología reparativa/cicatrización en la infancia -Capacidad de suplir con escaso volumen transfusional 1-2 veces -En el caso del páncreas, sus buenas condiciones primarias -Dificultades y dudas diagnosticas por imagen -Necesidad de mantener los órganos originales en la infancia: esófago, bazo…
