As a consequence of proteinuria, the intrarenal activation of the complement cascade may promote injury through the formation of membrane attack complex and biologically active products, such as C3a, that interact with specific receptors. Monocytes/macrophages contribute to fibrosis by release of TGF-β, which stimulates myofibroblast formation and collagen deposition and epithelial mesenchymal transformation. CONCLUSIONES Todos los pacientes con proteinuria persistente deberían ser evaluados por un nefrólogo: profundizar estudios, evaluar intervenciones y tratamiento, y seguimiento estricto
