2 GRIPE Gripe verdadera. Virus influenza A, B. (Infecciones virus influenza C, mucho menos graves) Enfermedad respiratoria febríl, con síntomas sistémicos causada por gran variedad de organismos llamada frecuentemente “gripe”.

21 Virion Maturation and Assembly
Capped mRNAs exported to nucleus are translated by ribosomes in the cytoplasm H, N, and M2 are folded and glycosylated and transported to the trans Golgi network and cell surface where assembly takes place. One copy of each genome segment is packaged into the virion.

22 Release of Influenza Virions
Virions are released by budding. The viral N protein cleaves the sialic acid on host cells to prevent clumping of viral particles at the host surface. Zamanivir and oseltamivir phosphate (sold as Tamiflu) inhibit the function of N.

28 PATOGENIA Respuesta inflamatoria : Monocitos y linfocitos, (edema submucoso). Producción de interferón y citocinas: Responsables síntomas sistémicos. Linfocitos T: curación/inmunopatogenia. Anticuerpos neutralizantes: curación e inmunidad (cepa específica). Frente a HA y NA (anti HA son más importantes. IgG e IgA: IgG menos efficaz pero más duradera.

Pérdida tejido tisular. Interferón: fiebre, fatiga, mialgias, malestar general Reparación tisular:puede ser larga

30 Cytokine Storm Also referred to as systemic inflammatory response syndrome (SIRS). SIRS may explain the devastating nature of the strain of influenza. The immune system overreacts toward the pathogen. Cytokines signal macrophages to travel to the site of infection, causing damage to the body and organ failure. H5N1 Avian Influenza virus also causes SIRS and is at least 50% lethal in humans.

31 Inmunidad Infected individuals develop antibodies against the outer proteins of Influenza virus—neuraminidase (N) and hemaglutinin (H) Antibodies against H neutralize the virus Antibodies against N do not but they do reduce the release of virus from infected cells

32 Inmunidad If a person has been infected in the past couple of years by a closely related strain of influenza H subtype: Their antibodies may intercept and neutralize the virus, protecting the lungs.

33 CUADROS CLÍNICOS Asintomática-grave.
Población de riesgo: niños pequeños, ancianos, inmuno-comprometidos, enfermedad cardiopulmonar. Síndrome gripal: Malestar/cefaleas. Fiebre,escalofrío y mialgias.Tos no productiva. Recuperación:7-10 días

34 COMPLICACIONES Pulmonares: No pulmonares: Croup (niños pequeños)
Neumonia primaria (virus Influenza). Infección secundaria bacteriana: Streptococcus pneumoniae, Staphlyococcus aureus, Hemophilus influenzae. No pulmonares: Miositis (rara A, > en niños, > con tipo B). Complicaciones cardiacas. Hígado y SNC: Síndrome de Reye. Sistema nervioso periférico: Síndrome Guillian-Barré.

35 IMPACTO DE LA GRIPE Principales causas de muerte asociada a gripe: neumonía bacteriana y fallo cardiaco. 90% de los que mueren son >65 años. Posibles factores que pueden influir en incremento morbilidad y mortalidad: Aumento edad media población. Aumento supervivencia pacientes cardiopulmonares y neonatos de alto riesgo. Aumento nº pacientes inmunodeprimidos.

36 DIAGNOSTICO Clínico + virus en la comunidad
Confirmación: Diagnóstico virológico Secreciones respiratorias Aislamiento del virus: células riñón de mono. Detección por hemadsorción. (hemaglutinación). Detección Ag (tipo A o B): IF, ELISA. Técnicas moleculares (subtipo): PCR Interés epidemiológico.

37 TRATAMIENTO Amantidine Rimantidine
First drugs: M2 inhibitors (prevent uncoating step) Amantidine Rimantidine New class of antivirals: N inhibitors (prevents neuraminidase from cleaving sialic acid during budding) Causes viruses to clump at the cell surface, reducing viral spread Oseltamivir Zanamivir (must be inhaled) Treatment must begin within 36 hours of onset of symptoms Used prophylactically in chronic care facilities
