So, I think we’re all familiar with the rapid progress of HCV therapy over the last number of years, and it really has been a dramatic change in the treatment landscape. And I think it’s an exciting time to be in this field, where we see treatments going from cure rates of under 10% with standard interferon through expectations of over 90% treatment efficacy now, with the most recently approved direct-acting antivirals in combination with peginterferon and ribavirin. 14 2015: Tractament hepatitis C a la Unitat d’Hemofília Vall d’Hebron
A principi de Gener 2015, es comencen a sol·licitar tractaments AADs a la Unitat d’Hemofília El primer tractament comença el 3 de Març de 2015 Actualment, ja s’han inclós 40 pacients: Sofosvuvir+Ledipasvir: 20 Sofosvuvir+Ledipasvir+Ribavirina: 8 Sofosvuvir+Simeprevir:4 Sofosvuvir+Simeprevir+Ribavirina: 4 Sofosvurir+Daclatasvir: 2 Sofosvurvir+Daclatasvir+Ribavirina: 2 Pendents d’autorització: 3 pacients

15 Característiques dels pacients en tractament amb AADs
El 80% dels pacients inclosos ja havien rebut tractament previ amb IFN+RBV Setze pacients (40% dels tractats) estan coinfectats per VIH S’han inclós pacients joves i pacients no tan joves S’han inclós pacients amb un cert grau de lesió hepàtica S’han inclós pacients que també tenen altres problemes de salut: Celiaquía, malalties autoimmunes, diabetes, processos oncològics recents, etc.

17 Lo millor… Tractament ben tolerat
Fàcil de prendre (via oral 1 o 2 cops al dia) Durada curta (12-24 setmanes)

18 Lo pitjor… Controls analítics i visites amb l’hepatòleg cada 4 setmanes (2 cops al mes) Cues a la Unitat de Dispensació de Farmàcia ??

22 Resum Per fi es disposa d’un tractament altament efectiu i ben tolerat contra el virus de l’hepatitis C En menys d’un any, 40 pacients de la Unitat d’Hemofília han rebut el tractament amb AADs 5 pacients han finalitzat el seguiment post-tractament i s’han curat El tractament amb AADs ha estat el resultat de molts anys d’investigació Els avenços tecnològics (Fibroscan, biologia molecular) també han contribuit a un millor control clínic dels pacients Per primera vegada, és factible la ERRADICACIÓ de la hepatitis C
