RESULTADOS Se incluyeros 58 pacientes. En el periodo A, la prevalencia de pacientes con Hb dentro de ámbito diana fue del 53,4%. El 37% de los pacientes presentan valores por encima del intervalo y el 5% no alcanzan los valores mínimos. En el periodo B, La prevalencia de pacientes con Hb dentro del ámbito óptimo es del 75,9%, con un 6% de los pacientes por encima y un 22% con valores por debajo. En el centro de referencia, en el periodo B, el 65,5% de los pacientes están dentro del intervalo diana, con un 29% por encima del límite y un 7% por debajo del límite. El incremento de porcentaje de pacientes dentro de ámbito diana del periodo A frente periodo B es 22,5% (IC95% 5,3%-39,6%) con Un Riesgo Relativo de 1,42 (IC95% 1,07-1,88). La diferencia de porcentaje dentro del ámbito diana del centro de estudio frente al centro de referencia es 12,4% (IC95% -3,4%-28,2”) con un riesgo relativo de 1,20 (IC95% 0,95-1,50). EVALUACIÓN DE LA MEJORA DE LA CALIDAD DE LA FARMACOTERAPIA EN PACIENTES CON ESTIMULANTES ERITROPOYÉTICOS EN HEMODIÁLISIS MATERIAL Y MÉTODOS Tipo de estudio: transversal analítico con medidas preintervención frente postintervención y centro de estudio frente centro de referencia. Pacientes: todos los pacientes en tratamiento con Factor Estimulantes eritropoyeticos (FEE) y con Insuficiencia Renal Crónica (IRC) en hemodiálisis. Periodo de estudio: periodo A (marzo 2006) previo a la implantación del protocolo y periodo B (marzo 2007) posterior a la implantación. El protocolo para el tratamiento con FEE se consensuó e implantó en marzo 2006 entre el Servicio de Farmacia y Nefrología y recoge los siguientes parámetros: tipos de FEE (eritropoyetina.beta o darbepoetina-alfa), dosis inicio/mantenimiento e intervalo posológico. Variables analizadas: valores de Hemoglobina (Hb) y proporción de pacientes con Hb (g/dL) dentro/fuera de un óptimo diana (10,5-13 g/dL). Análisis estadístico: diferencias de porcentaje de pacientes dentro de ámbito diana de hemoglobina e intervalo de confianza 95% y Riesgo Relativo con su intervalo de confianza del periodo A frente al periodo B y del centro de estudio frente al centro de referencia. CONCLUSIÓN La implantación de un protocolo normalizado de trabajo con FEE en pacientes con anemia secundaria a IRC en hemodiálisis aumenta significativamente la prevalencia de pacientes con valores de hemoglobina dentro del ámbito diana, para garantizar la efectividad y seguridad del tratamiento.
