2 Antecedentes Del 5-6% de los px con PortHTN tienen PVR Cirrosis  GC y RVP P Vasculatura Pulmonar  P. VD  PVC  Congestión Hepática Factores de Riesgo Cardiomiopatia cirrotica Insuf.Cardiaca Sobrecarga de Volumen PVR Portopulmonary hypertension and right heart failure in patients with cirrhosis

3 PHTN ICD – PHTN – Sobrecarga de volumen – Cardiomiopatia Cirrotica – Disminuye contractibilidad Cambios Vasculares Pulmonares – Vasoconstricción  reversible  vasodilatadores – Fibrosis  Irreversible Portopulmonary hypertension and right heart failure in patients with cirrhosis

4 PPHTN DEFINICIÓN – PHTN + PortHTN sin o con cirrosis (>mortalidad) Dx Criterios – PAPm > 25mmHg – PVR > 240dyn.s.cm-5 – PVR  +IMPORTANTE Requiere cateterización card. Derecha. Portopulmonary hypertension and right heart failure in patients with cirrhosis

5 PPHTN Grado – Leve: PAPm 25-35 mmHg – Moderada: PAPm 35-45 mmHg – Severa: PAPm > 45 mmHg Factores que afectan sobrevivencia – Edo. VD – Compliance de vasculatura pulmonar – Reversibilidad con vasodilatadores Portopulmonary hypertension and right heart failure in patients with cirrhosis

6 Monitoreo y Evaluación 1.Ecocardio Transtorácico (TODOS) Transesofágico durante la cirugía 2.PAPm < 35 mmHg Buena fx VD y responde a vasodilatador  Lista de transplantes 3.PAPm 35-45 mmHg Muy buena Fx VD y 6 m de vasodilatador  transplante 4.PAPm >45 mmHg Revertir a < 35 mmHg y eco cada 6 m p/ evaluar estado Portopulmonary hypertension and right heart failure in patients with cirrhosis

7 PPHTN …Pacientes con un aumento progresivo de las PVR presentan cambios positivos en la musculatura del VD, permitiendo una mayor respuesta al aumento del GC, PVR y sobrecarga de volumen… Portopulmonary hypertension and right heart failure in patients with cirrhosis

8 Monitoreo PPHTN VD Cateterización ECO Prueba con sobrecarga de líquidos y dobutamina Esenciales para determinar la respuesta del paciente durante la cirugía Portopulmonary hypertension and right heart failure in patients with cirrhosis

9 Terapéutica Diuréticos – < PHTN y PVC pero no PVR O2 – satO2 > 90% Epoprostenol (prostenoide) – fortalece el VD – IV Bosentan (Endotelina Bloq.) – VO Portopulmonary hypertension and right heart failure in patients with cirrhosis

10 Terapéutica Sildenafil (I. Fosfodiesterasa 5 – < PHTN y PVR Oxido Nitrico – Durante el Trans para manejar las presiones pulmonares durante la fase de reperfusión Portopulmonary hypertension and right heart failure in patients with cirrhosis

11 Conclusiones La causa de la PPHTN no es totalmente conocida La efectividad del un transplante es controversial para mejorar la PPHTN, excepto en pacientes respondores a terapia farma… Existen reportes de PPHTN de novo en pacientes postransplantados

12 Conclusiones …LA FUNCIÓN VENTRICULAR DERECHA ES EL CRITERIO MAS IMPORTANTE PARA OBTENER UN TRANSPLANTE EXITOSO… …EL ÉXITO DEL TRANSPLANTE SERA DETERMINADO POR LA CAPACIDAD DEL VD PARA MANEJAR AUMENTOS DE PVR, VOLUMEN Y GC, SIN DESARROLLAR INSUFICIENCIA ….SI OCURRE….>PVC CONGESTIÓN DEL ORGANO Y …FALLA… Portopulmonary hypertension and right heart failure in patients with cirrhosis
