PCC focuses on process improvement in 3 areas or domains: Care Practices, Workplace Practices, and Environment. Each of the three domains have guiding principles. You’ll notice that each area overlaps, this overlapping exemplifies the PCC philosophy that the person is more important than the system and that the systems should support the people who live and work in the NH.

Under Care Practices, the focus of the care shifts from the traditional NH model of the Institution knows what’s best for the resident toward climate that honors and embraces individual choice. For example, under the medical model, residents are gotten up and served breakfast each morning at a set time. In PCC, a person may decide to sleep in and eat later. Systems produce outcomes. Honoring individual choice produces desirable outcomes promoting optimal level of function for all.

8 Ambiente Confíar en mejorar los alrededores para promover un sentido del hogar y comunidad. El ambiente debe apoyar las relaciones entre los dominios. Un ambiente hermoso no iguala la calidad del cuidado o la calidad de vida. A sense of home and community cannot be made by changing the environment alone. The environment must support the the relationships between the domains. A beautiful environment does not equal quality of care or quality of life. There is often a trend to start with the environment, redecorate the lobby, add a few plants or pets, and say we’ve done culture change. These changes are visible and tangible but making physical changes alone only adds to the number of artifacts in the surroundings, not a sense of belonging or security. Care giving is an art, the environment must support that relationship between the caregiver and care recipient. Do people in nursing homes have a sense of home? Are they surrounded by what reminds them of home and community? Or, are they in a way homeless--- void of everything and everyone that represents home to them?
