ENROLLMENT REQUIREMENTS The parent or guardian of the enrolling student must bring the following documents in order to complete the enrollment process. STUDENTS NEW TO LAUSD 1. Proof of identification and verification of address is required (residential only). a. Current DMV identification or Drivers License. b. Current/original DWP/Gas bill with the current address and the name of the parent or guardian enrolling the student. (You may bring a Residential Service Confirmation if your account was just opened). If utilities are listed as included on lease/contract, please provide a bank statement with your name and address. Photocopies will not be accepted. (Telephone, Cable, Cell Phone bills, etc will not be accepted) c. Original Deed or Escrow Papers, Rental/Lease Agreement with the current address and the name of the parent or guardian enrolling the student. Photocopies will not be accepted. 2. Age Verification of the Student: Birth Certificate, Baptism Certificate or Passport. 3. Immunization Records (this is the yellow immunization card) which includes Mantoux (TB) test and results, Hepatitis B (3 Doses) and MMR (2 Doses). 4. Out of district students are required to bring transcripts from the previous school of attendance. Please have previous school contact information available. 5. Final report card from the previous school. STUDENTS FROM ANOTHER LAUSD SCHOOL 1. PAR, the Pupil Accounting Report issued from the previous school of attendance /school assigned to. 2.
