ENROLLMENT REQUIREMENTS The parent or guardian of the enrolling student must bring the following documents in order to complete the enrollment process. STUDENTS NEW TO LAUSD 1. Proof of identification and verification of address is required (residential only). a. Current DMV identification or Drivers License. b. Current/original DWP/Gas bill with the current address and the name of the parent or guardian enrolling the student. (You may bring a Residential Service Confirmation if your account was just opened). If utilities are listed as included on lease/contract, please provide a bank statement with your name and address. Photocopies will not be accepted. (Telephone, Cable, Cell Phone bills, etc will not be accepted) c. Original Deed or Escrow Papers, Rental/Lease Agreement with the current address and the name of the parent or guardian enrolling the student. Photocopies will not be accepted. 2. Age Verification of the Student: Birth Certificate, Baptism Certificate or Passport. 3. Immunization Records (this is the yellow immunization card) which includes Mantoux (TB) test and results, Hepatitis B (3 Doses) and MMR (2 Doses). 4. Out of district students are required to bring transcripts from the previous school of attendance. Please have previous school contact information available. 5. Final report card from the previous school. STUDENTS FROM ANOTHER LAUSD SCHOOL 1. PAR, the Pupil Accounting Report issued from the previous school of attendance /school assigned to. 2. Current DMV identification or Drivers License. 3. Final report card from the previous school. 4. ADDRESS CHANGES (residential only) will require (a) and (b) below, along with above items that apply: a. Current/original DWP/Gas bill with the current address and the name of the parent or guardian enrolling the student. If utilities are listed as included on lease/contract, please provide a bank statement with your name and address. Photocopies will not be accepted. (Telephone, Cable, Cell Phone bills, etc will not be accepted) b. Original Deed or Escrow Papers, Rental/Lease Agreement with the current address and the name of the parent or guardian enrolling the student. Photocopies will not be accepted. SAS only: These are parents that have received confirmation by mail of acceptance to JB. Please bring your acceptance letter along with the documents requested in this section. The application deadlines for these permits have passed and no other permits will be granted. IEP only: If the student has an IEP, please bring a current copy of the IEP and enrollment documents. The enrollment process will be completed by appointment at a later date. Cuenta original de DWP o Gas actual con la dirección actual y el nombre del padre o guardián del estudiante. Si las utilidades están incluidas en el contrato, una cuenta de banco con su nombre y dirección se recibirá. Fotocopias no serán aceptadas. (Cuentas de teléfono, cable, celular, etc. no serán aceptadas) B. Papeles originales de la hipoteca o del contrato de renta que tenga el nombre del padre o del guardián del estudiante. Fotocopias no serán aceptadas. 3. VERIFICACION DE LA EDAD DEL ESTUDIANTE: (Opción A, B, o C) A. Partida de nacimiento B. Pasaporte C. Certificado de bautismo 4. EXPEDIENTE DE LA INMUNIZACIÓN (tarjeta amarilla) que incluye la prueba de Mantoux (TB) y resultados, hepatitis B (3 dosis) y MMR (2 dosis). 5. TRANSCRIPCION: Estudiantes deben de traer transcripciones (récords escolares) de la escuela anterior. Tengan a la mano información de contacto de escuela previa. 6. BOLETA DE CALIFICACIONES final de la escuela anterior. ESTUDIANTES DE OTRA ESCUELA DE LAUSD 1. P.A.R. o ID20 (el informe de la contabilidad del estudiante) de la escuela anterior/escuela asignada. 2. IDENTIFICACION actual de los padres: (Opción A, B, o C) A. I.D. de DMV/Licencia de conductor B. Pasaporte C. Matricula Consular 3. BOLETA DE CALIFICACIONES final de la escuela anterior. 4. PRUEBA DE RESIDENCIA (de hogar): Cambios de dirección asignada a nuestra escuela requerirá (A) y (B) de abajo y los requisitos de arriba que aplican: A. Cuenta actual/original de DWP/Gas con la dirección actual y el nombre del padre o guardián del estudiante. Si las utilidades están incluidas en el contrato, una cuenta de banco con su nombre y dirección se recibirá. Fotocopias no serán aceptadas. (Cuentas de teléfono, cable, celular, etc. no serán aceptadas) B. Papeles originales de la hipoteca o del contrato de renta que tenga el nombre del padre o del guardián del estudiante. Fotocopias no serán aceptadas. SAS solamente: Este es para padres que recibieron confirmación de aceptación por correo. Por favor de traer la carta con los documentos requeridos en esta sección. Los plazos para estos permisos han pasado y no se concederá ningunos otros permisos. IEP solamente: Si el alumno tiene un IEP, por favor de traer una copia al día del IEP con los otros documentos necesarios; registración para alumnos con IEP se completara por cita en una fecha asignada. www.burroughsms.org
