3 Marcapasos Dispositivo electrónico diseñado para producir impulsos eléctricos con el objeto de estimular el corazón cuando falla la estimulación fisiológica o normal. Estos impulsos, una vez generados, necesitan de un cable conductor (o electrocatéter) que se interponga entre ellos para alcanzar su objetivo. De esta forma, un sistema de estimulación cardiaca consta de un generador de impulsos eléctricos (o marcapasos propiamente dicho) y de un cable. 3

4 Tipos Marcapasos temporales: el generador no está implantado en el paciente, y pueden ser: Transcutáneos (generalmente incluidos en algunos desfibriladores): los electrodos se colocan sobre la piel, uno en la parte anterior del tórax (electrodo negativo) y otro en la espalda (electrodo positivo). Intravenoso (endocavitario): los electrodos son colocados a través de una vena central hasta contactar con el endocardio. Marcapasos permanentes: el generador se implanta subcutáneamente. 4

5 “ 5  Se recomienda la TRC-P en pacientes en clase funcional III- IV de la NYHA que permanecen sintomáticos a pesar de tratamiento médico óptimo y que tienen una fracción de eyección disminuida (FEVI ≤ 35%) y prolongación QRS (ancho QRS ≥ 120 ms) para reducir la morbimortalidad.  Se recomienda la TRC con desfibrilador implantable (TRC-D) en pacientes en clase funcional III-IV de la NYHA que permanecen sintomáticos a pesar de tratamiento médico óptimo y que tienen una fracción de eyección disminuida (FEVI ≤ 35%) y prolongación QRS (ancho QRS ≥ 120 ms) para reducir la morbimortalidad.
